"Consent to Join" Form
If you worked for Battelle as a Customer Service
Representative or Agent, you may be eligible to receive
unpaid wages and overtime. If you are interested in
joining the class action lawsuit so that you can share in
any recovery of unpaid overtime wages, you must send in a
completed, signed "Consent
to Join"
form. Simply fill out this consent form and return it to the
address listed below.
Battelle
employees who do not join in the lawsuit by filing the
consent form will not be allowed to share in the recovery of
any unpaid federal overtime wages.
